Disability Support Team

The Disability Support Team offers a personalised service to people with complex or severe physical impairment to help them achieve a full and purposeful life. The service is available to disabled people who:

  • Are aged 16-64 years
  • Are in receipt of Disability Living Allowance
  • Have a complex/severe physical impairment
  • Have a progressive medical condition e.g. MS, MND, Parkinson’s Disease

Patients are allocated to a clinical specialist who will discuss issues that the patient has identified as being important to them. The clinical specialist will use their expertise, extensive contacts, knowledge of the services available and a problem-solving approach in order to establish agreed and shared goals.

Patients with an acquired brain injury will be allocated to the Acquired Brain Injury Case Manager who can provide specialist intervention to address the needs of their ‘hidden’ disability.

The service works in partnership with Health and Social Services to ensure clients receive a co-ordinated, community rehabilitation programme.

The Disability Support Team is a community-based service and therefore patients are usually seen in their homes and treated in the community.
