The service helps to improve the health and well being of people with Chronic Obstructive Pulmonary disease (COPD) and associated respiratory conditions by assisting patients to self-manage their respiratory condition.

Patients can be referred by GP, Nurse or Health Care Assistant or by the hospital.

• Monday to Friday – 08.00 to 19.00
• Saturdays, Sundays and bank holidays 09.00 to 17.00
