On page 30 there is a declaration to be signed confirming you have read and understood the contents of the module.
BANK ONLY STAFF (CSWs/RNs)
If you are a BANK ONLY clinical Support Worker or Registered Nurse your signed declaration must be sent to the Staff Bank Training Department;
Post: Staff Bank Training, Learning and Development Department, Clinical Education Centre, Russells Hall Hospital, Dudley, DY1 2HQ or provide a fully completed and signed scanned copy to dgft.staffbank.training@nhs.net
