The Dudley Health and Care Partnership is made up of local organisations including the NHS, council, voluntary and community sector. We work together to plan, commission and deliver co-ordinated services for the people of Dudley – building better health and care services.

Why do we need Dudley Health and Care Partnership ?

Dudley Health and Care Partnership is hosted by Dudley Group NHS Foundation Trust. Although Dudley Health and Care Partnership is not a statutory organisation, there is a strong emphasis on strengthening the way in which local organisations work together and they are often referred to as place-based partnerships. We recognise that by working together, we can have the greatest impact on tackling issues that cause some of the biggest problems in our local communities which contribute towards health inequalities. We also know that it is important to listen to and work closely with our communities to help shape services that can make a difference and to understand what the issues are.

 

What are health inequalities?

Health inequalities are avoidable and unfair differences in health between different groups of people. These differences are often linked to factors like income, education, where people live, and their social or cultural background. For example, some people may not have access to healthy food, green space, or safe living conditions, which can make them more likely to get sick or die earlier than others. These inequalities can also be influenced by things like race, gender, or disability. Essentially, health inequalities happen when some people have better opportunities to live healthier lives than others.

 

What are our priorities?

Our priorities are: · Strengthening the impact and effectiveness of our partnership working · Transforming the experience for citizens when accessing support, help and information · Shift the curve of future demand taking actions to influence and change the way that people currently use and access services and help · To reduce inequalities in health and healthcare · To improve outcomes for children and young people We will achieve our priorities by focusing on · Delivering care closer to home with better outcomes for patients · Improving our patient journey pathways through greater integration and collaboration of our teams · Giving citizens more say in their care through coproduction of health and care in Dudley · Supporting our voluntary and community services to thrive · Supporting our workforce to stay well in work and recruiting staff who are representative of the population we serve.

What is Dudley Health and Care Partnership trying to achieve?

For children and young people

· An increase in the number of children with good emotional and personal development at the end of their reception years

· A decrease in obesity rates for our Year 6 children

For all patients

Patients who are discharged from hospital are not readmitted to hospital within 91 days and instead are comfortable and supported in their home environment

· An increase in the number of people who die in their preferred place of death For mental health · An increase in employment levels for patients who have a mental health condition · An increase in the number of people who are in contact with secondary care mental health services who live independently Cancer · An increase in early cancer diagnosis · An increase in cancer survival rates which are longer than 12 months Long term conditions · A decrease in the number of people living with a long term condition who die in the winter months Health inequalities · An increase in the number of people who are Dudley residents being employed by our partner organisations

What are some of our key achievements in the last year? ·

We have family hubs in our local townships and they support our children and young people and families on a range of issues that are important to them · We have secured funding for our voluntary, community and faith organisations to promote a number of initiatives that support health and wellbeing · Our Clinical Hub is developing and expanding which helps in avoiding attendances at the Emergency Department, ensuring people get the right care at the right time · We are supporting inclusive recruitment through iCan which sees employment into health and care organisations locally · We have 11 Community Partnership Teams (CPTs) based across our 6 primary care networks that work together to wrap care around the patients that need it most · Our Healthy Hearts Hub is a community-based initiative which promotes healthy hearts such as knowing your blood pressure, and positive lifestyle choices
