To complete Patient Falls Prevention Training, read the leaflet following the link below.
On pages 25-29 there is a questionnaire and declaration to be completed. The declaration is to confirm that you have read and understood the contents of the leaflet and have completed the questionnaire. You must achieve 20 out of 25 to pass as complete.
BANK ONLY STAFF (CSWs/RNs)
If you are a BANK ONLY Clinical Support Worker or Registered Nurse your completed questionnaire and declaration must be sent to:
