Patient Safety Incident Response Framework (PSIRF)

Every day more than a million people are treated safely in the NHS. Occasionally, things go wrong, or an unexpected event occurs. These are known as patient safety incidents.

A patient safety incident is any unintended or unexpected incident which could have, or did, lead to harm for one or more patients receiving healthcare, and can range in scale from the most minor to the other end of the scale.

To help ensure we learn and improve following a patient safety incident, and to work better with parents and families when things go wrong, we have implemented the national Patient Safety Incident Response Framework (PSIRF). Within this Framework we will use a variety of ways to identify learning and make improvements following an incident where a patient was or could have been harmed.

Guidance and support

Patients often provide an invaluable perspective to the circumstances around incidents. We value this perspective and know this will strengthen our understanding of events and subsequent improvement work.  We are therefore building meaningful engagement processes as well as our support offer as we transition to PSIRF.
