What is a Virtual Ward? 

Virtual Wards allow patients to receive hospital-level care safely in the comfort of their own home. Being in familiar surroundings can help people recover more quickly, while also freeing up hospital beds for those who need them most. 

How does it work? 

Our dedicated team of Doctors, Advanced Clinical practitioners (ACPs), and Pharmacists—supported by specialist hospital consultants—provide expert care to help you stay well at home. 

The team offers a wide range of tests and treatments, including blood tests, intravenous medications, and fluid replacement therapy. 

As a patient on the virtual ward, you will be reviewed daily, with your care discussed in a Multidisciplinary Team meeting led by a specialist doctor to ensure the best possible care and support is received. 

We work closely with hospital specialists across a wide range of conditions to ensure you receive expert, personalised care. This includes support for: 

  • Acute medicine 
  • Diabetes 
  • Frailty 
  • Heart failure 
  • Palliative care 
  • End of life 
  • Renal conditions 
  • Respiratory conditions 
  • Oncology (cancer care) 

We also provide onward referral to appropriate in-hospital teams and community services where needed, ensuring a full and holistic approach to your care.

How to access the service:

Referral process 

  • Patients may be referred from ED, AMU, SDEC, medical wards, or via the Care Navigation Centre, but must have received at least one post-take consultant review. 
  • Referred to the named Virtual Ward Consultant for face-to-face assessment. The consultant will confirm clinical suitability.  
  • Referrals should be made between 8am – 3pm. Referrals after 3pm will be reviewed in hospital the next morning.
