The Clinical Pathways Hub is your single, trusted source of up-to-date local clinical guidance. It is designed to help ensure patients receive the right care, at the right time, in the right place – first time. We have a number of services that provide pathways of care that can be accessed directly by patients, or via a healthcare professional. Our pathways are developed in collaboration with clinicians across primary and secondary care. They reflect shared expertise from those who refer, request investigations, and provide specialist advice and patient reviews.
The CNC is one of the first points of call for patients, GPs and healthcare professionals and offers a range of services.
